We believe in 100% full continuity of care. We are bringing back old fashioned continuity of care medicine with all of the benefits of modern day medicine. We offer Urgent Care, Primary Care, and we also offer Primary Care house calls if it is hard for you to leave home.
Our excellent care does not end if you need to go into the hospital. We do not hand our patients over to the hospitals group who know nothing about you. We are one of only a few Primary Care groups in San Diego that admit our own patients to Grossmont Hospital. If you need rehab / skilled nursing, we follow you there as well. If you require admission to a hospital or skilled nursing facility, this is NOT the time to break continuity of care and involve several other doctors. This is when you need our doctors. This is when you need our continuity of care.
We arrange in home physical therapy, home health, hospice, and durable medical equipment. We communciate with your specialists so we are on the same page. We see our patients in the clinic, in the hospital, skilled nursing facilities, assisted living facilities, and board & care homes. We are with you no matter what, no matter when.
